How to Register
The quickest way to register at the Practice is to register online. This process will usually take up to 10 days to get you onto the system. You will receive a confirmation message to your preferred contact method once you are fully registered. If you have not received this message after 2 weeks please contact the practice as there may be missing information on your forms.
You can also register with the practice by coming in to the practice in person. If you choose to come in to the practice New Registration forms are available to be downloaded and completed at home prior to coming in. New patients are also asked to complete a Health Questionnaire which will be followed up with an appointment with one of our Health Care Assistants for a New Patient Health Check.

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.

Additional Information
Please tell us about any regular medication you take. It is very helpful if you bring in a repeat prescription print-out from your previous surgery. You will need to see one of the GPs before any repeat prescriptions can be issued, so we will try to book you an appointment with a GP after seeing the Health Care Assistant.
Please ensure all questions are answered in full, any gaps may results in your registration with the practice being delayed.
Please allow 5 working days for your registration to be processed. Once the registration process is complete, we can apply to your old Surgery for the medical records.
Acceptance on to the list is as at the discretion of the Doctor and we may ask you for proof of identity. Please note that your medical records may take several weeks to arrive at the Practice.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
The National Care Record Service (NCRS)
Important information regarding your NHS medical records – Summary Care Record
The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

Organ Donation
